Participation Rules for the Frederick Wellness Challenge
Only participants, who register, pay the registration fee, get their initial & final Creating Wellness Assessment, and meet the participation guidelines will be eligible to win any prize being issued by the Frederick Wellness Challenge.
Referral Rebates: Referrals are defined as an individual who has never participated in a Frederick Wellness Challenge Event, who has registered and paid their enty fee, and has named their referral source by name and email in their registration form. The rebate amount is $20.00 per referral. The person you refer must indicate your name and email address on their registration form. Failure to do so at the time of registration will eliminate you from being named as the referral source, therefore making you ineligable to receive that referral rebate. There will be no exceptions made to this rule. Once your name and email are received from a referred competitor, you will receive a confirmation email. There will be a limit of 5 referral rebates per competitor. Each referral rebate is worth $20.00 with a maximum rebate of $100.00 possible. Rebate checks will be issued within 2 weeks of the start of the competition.
All Participants must have their Creating Wellness Assessment done during the designated dates to be eligible to compete.
All Participants must have a final Creating Wellness Assessment done during the designate dates to be eligible for any prize issued to the winners of the competition.
Participants are not required to participate in all Frederick Wellness Challenge Events or activities in order to be eligible for prizes. However, they are strongly encouraged to take advantage of as many opportunities as possible.
You cannot use surgical means to reduce your weight or improve your Challenge Quotient.
Participants must be 18 years or older.
No Creating Wellness Alliance or Creating Wellness Center employees and their family members are eligible for the Grand Prize.
Winner Selection is based on the greatest percentage of improvement of participant's Challenge Quotient. The Challenge Quotient and Wellness Quotient are determined by your Creating Wellness Assessment.
Decision of the winners will be final as per Frederick Wellness Challenge Committee.
Contestants agree to submit a testimonial about the experience if asked and be available for interviews with radio, TV, and newspaper as well as photo opportunities with the Frederick Wellness Challenge Team, Frederick's Mayor, and the Sponsors.
In the case of a tie we will base the decision of winner off of greatest level of participation and most improvement in the areas of weight, body fat percentage, and girth.
Registration Fee is Non-Refundable
Decrease of weight or girth as a result of giving birth would make a contestant ineligible for prizes given by the Frederick Wellness Challenge.
Participants understand and agree that implementing any advice given during the competition, whether it is in regards to fitness, nutrition, or otherwise, is at the sole discretion of the participant as it may not be in their best interest with regards to the participant’s health history, known allergies, etc.
If there is a pre-existing condition it is the participant’s responsibility to clear their participation with their Doctor prior to joining the competition.
Participants agree to not hold The Frederick Wellness Challenge, it's employee's, or sponsors responsible for any injury or illness that may occur as a result of participation in the competition.
